FINANCIAL OPTIONS
AM Training Institute accepts the following methods of payment: Employee Assistance, Central Iowa Works or Iowa Vocational Services (students with disabilities) by vouchers, Debit/Credit and cash/check. All deposits, down payments become part of
tuition and fees.
Standard Plans :
- Payment In Full = $680.00 ($660.00 cash or check)
- $340.00 ($330.00 cash or check) ½ Down and $340.00 ($330.00 cash or check) due by the last day of class and before receiving the certificate and being reported to the Dept of Inspection & Appeals for certificate verification.
- Voucher (Central Iowa Works or IVS)
Additionally students need to purchase scrubs. It must be gray or black pants and a yellow top. We do offer this uniform at our facility for $40.00.
For those choosing the ½ down option:
Students who are delinquent on a scheduled payment will receive a notice of their account, reminding them that a payment was due. If payment is not received within 7 days of the original notice, a second notice is sent. Failure to pay 14 days
after the initial notice is sent will result in a student being dropped from the course. If a student is dropped from a course, he/she may reapply for admission after his/her account has been reconciled.
In order to be accepted students must also provide the following items:
- CPR certification: We provide this through AM Training Institute for a fee of $65.00
- Negative TB test results (provided at a separate location for a fee of $25.00)
- Current ID
- Social Security Card
- Covid Vaccination Record (If not vaccinated student is required to fill out an exemption form and be tested each clinical morning, by nose swab)
Notice to Student
- Do not sign this agreement before you have read it or if it contains any blank spaces
- This agreement is a legally binding instrument and is only binding when the agreement is accepted, signed, and dated by the authorized official of the school or the admissions officer at the school’s principal place of business. Read all pages of this contract before signing.
- You are entitled to an exact copy of the agreement and any disclosure pages you sign.
- This agreement and the school syllabus constitute the entire agreement between the student and the school.
- Any changes in this agreement must be made in writing and shall not be binding on either the student or the school unless such changes have been approved in writing by the authorized official of the school and by the student or the student’s parent or guardian. All terms and conditions of the agreement are not subject to amendment or modification by oral agreement.
- The school does not guarantee the transferability of credits to another school, college, or university. Credits or coursework are not likely to transfer.
Student Acknowledgments
- I hereby acknowledge receipt of the school’s catalog, which contains information describing programs offered, and equipment or supplies provided. The school catalog is included as part of this enrollment agreement and I acknowledge that I have received a copy of this catalog
- I have carefully read and received an exact copy of this enrollment agreement.
- I understand that the school may terminate my enrollment if I fail to comply with attendance, academic, and financial requirements or if I fail to abide by established standards of conduct, as outlined in the school enrollment agreement/syllabus. While enrolled in the school, I understand that I must maintain satisfactory academic progress as described in the school syllabus and that my financial obligation to the school must be paid in full before a certificate or credential may be awarded.
- I understand that the school does not guarantee transferability of credit and that in most cases, credits or coursework are not likely to transfer to another institution. In cases where transferability is guaranteed, AM Training Institute must provide me with copies of transfer agreements that name the exact institution(s) and include agreement details and limitations.
- I understand that the school does not guarantee job placement to graduates upon program completion.
- I, the parent and/or legal guardian of the candidate have read all information above regarding the 75-hour CNA training program conducted and I agree to all terms and conditions.
The Student acknowledges receiving a copy of this completed agreement, the school catalog, and written confirmation of acceptance prior to signing this contract. The student by signing this contract acknowledges that he/she has read this contract, understands the terms and conditions, and agrees to the conditions outlined in this contract. It is further understood that this agreement supersedes all prior or contemporaneous verbal or written agreements and may not be modified without the written agreement of the student and the School Official. The student and the school will retain a copy of this agreement.
The Parent acknowledges receiving a copy of this completed agreement, the school syllabus, and written confirmation of acceptance prior to signing this contract.
